Computer Rental Agreement. 1. Parties. This Agreement is made on [date] between [name], a [state] company with its principal office at [address] (Lessor), and [name], a [state] company, small business or other entity with its principal address at [address] (Lessee).

2. Description of Computer Equipment. Lessor will supply the following computer equipment (Equipment) to Lessee, and maintain and service it in accordance with this Agreement:
